ORDINANCE N0.2008-28 <br />INTRODUCED BY: Mayor Rinker and Council as a Whole <br />AN EMERGENCY ORDINANCE <br />AUTHORIZING AN AGREEMENT WITH GUARDIAN <br />AS AND FOR AN EMPLOYEE AD&D, LIFE <br />AND DENTAL PROGRAM. <br />WHEREAS, the Finance Committee has recommended and it is the <br />desire of Council to authorize an AD&D, life and dental plan for employees with <br />Guardian pursuant to the terms set forth in Exhibit "A" attached hereto and <br />incorporated herein by reference. <br />NOW, THEREFORE, BE IT ORDAINED BY THE COUNCIL OF THE <br />VILLAGE OF MAYFIELD, COUNTY OF CUYAHOGA (the "Entity") AND <br />STATE OF OHIO THAT: <br />SECTION 1. The Mayor is hereby and herein authorized and directed to <br />enter into an agreement with Guardian for AD&D and life pursuant to the terms, <br />conditions and costs as set forth in Exhibit "A" effective September 1, 2008 <br />through September 1, 2009, and for an employee dental program as stipulated <br />therein, at the rate of $35.73 per month per employee for single coverage, <br />$81.37 per month for employee and spouse, $102.77 per month for employee <br />and children, and $148.43 per month for employee, spouse and children. <br />SECTION 2. The Council finds and determines that all formal actions of <br />this Council relating to the adoption of this Ordinance have been taken at open <br />meetings of this Council; and that deliberations of this Council and of its <br />committees, resulting in such formal action, took place in meetings open to the <br />public, incompliance with all statutory requirements including the requirements <br />of Section 121.22 of the Ohio Revised Code. <br />SECTION 3. This Ordinance is hereby declared to be an emergency <br />measure immediately necessary to maintain the employee AD&D, life and <br />dental program for the employees of Mayfield Village, Ohio which is due to <br />expire on August 31, 2008. It shall, therefore, take effect immediately upon the <br />passage by the affirmative vote of not less than five (5) members elected to <br />
