RESOLUTION N0.2014-25 Page 2 of 3 SECTION 3. That the renewal tax levy shall be effective and shall be placed on the tax list and duplicate for each of the tax duplicate years of 2015, 2016, 2017, 2018 and 2019, if a majority of those voting on the question vote in favor thereof. SECTION 4. The Clerk of Council is hereby authorized and directed to certify a copy of this Resolution to the Cuyahoga County Board of Elections as provided by law no later than August 6th, 2014, and to notify the Board of Elections to cause notice of said election on the question of levying said tax to be given as required bylaw. SECTION 5. The Cuyahoga County Fiscal Officer has certified to the Village pursuant to Section 5705.03 of the Ohio Revised Code, the current total tax valuation of the Village and the dollar amount of revenue that would be generated by the renewal of 2.3 mill of an existing levy of a five (5) year period of time beginning with the 2015 tax list and duplicate. SECTION 6. The Board of Elections is hereby directed to certify the result of said election, immediately after the canvass of said Board of Elections, to the taxing authority of said County in order to permit the renewal of such levy, if approved by the electorate, on the 2015 tax duplicates for the electors of the Village. SECTION 7. The form of the ballot to be used at such election shall be substantially as follows: BALLOT LANGUAGE Mayfield Village, Ohio A Majority Affirmative Vote is Necessary for Passage "A RENEWAL OF A TAX FOR THE BENEFIT OF HAYFIELD VILLAGE FOR THE PURPOSE OF CURRENT EXPENSES AT A RATE NOT EXCEEDING 2.3 MILLS FOR EACH ONE DOLLAR OF VALUATION, WHICH AMOUNTS TO TWENTY-THREE CENTS ($.23) FOR EACH ONE HUNDRED DOLLARS OF VALUATION FOR FIVE (5) YEARS, COMMENCING 1N JANUARY 2015, FIRST DUE IN CALENDAR YEAR 2016." SECTION 8. Based upon this legislation, Council hereby repeals and fully rescinds Resolution No. 2014-19 passed July 21, 2014 and Resolution 2014-23 passed August 4th, 2014.
