Brett Director of Finance and Economic Development Mayfield Village 6621 Wilson Mills Road Mayfield Village, Ohio 44143 Subject: CUY-91-13.29 - SOM Center Road Change Order No. 2 and Extra Work Order No. 2 Letter No. 047 Dear Mr. Brett: The Great Lakes Construction Company has submitted the appropriate back-up documentation for Change Order No. 2 and Extra Work Order No. 2. Both Change Order No. 2 and Extra Work Order No. 2 are associated with three issues further explained below. ISSUE No. l -ADD SLEEVES FOR IRRIGATION CONDUIT FOR MEDIAN ISLANDS. The Village requested sleeves be placed under the pavement to allow the installation of irrigation conduit for the median island. The sleeves were installed to allow the irrigation conduit to be installed at a later date without .disturbing the recently improved roadway pavement. A similar request was made last year for the Phase 1 pavement and was incorporated into Change Order No. 1. This modification will provide access to the medians for the remainder of the project and results in an addition to the contract value of $35,800.00. ISSUE No. 2 -ADDITIONAL INNERDUCT CONDUIT AND ACCESS POINTS. The Village requested additional access be made available to the innerduct systems detailed in the contract documents. The Contractor has submitted a price for this additional work. This change to the contract documents creates an addition to the contract value of $10,760.75. The additional ITEM 625 -CONDUIT, MISC.: CONDUIT, 4" WITH 4 CELL INNERDUCT, TYPE 713.07 amounts to $1,500.00. The additional ITEM 625 -TRENCH amounts to $1,080.00. The additional ITEM 625 -PULL BOX, 713.08, 24" amounts to $4,970.00. The previously noted three items are Reference No. 193, No. 194, and No. 196 respectively. These items are accounted for by Change Order and contract extension of quantities. The additional cost associated with this work and the non:contract items are accounted for in the Extra Work Order. ITEM 625 -CONDUIT, MISC.: CONDUIT, 4" WITH 4 CELL INNERDUCT, TYPE 713.07 accounts for $927.00 in Extra Work. The additional work for the CONDUIT JACKED OR DRILLED UNDER PAVEMENT, 4" WITH 3-CELL INNERDUCT, 713.04 OR 713.07 SCHEDULE 80 accounts for $2,283.75 in Extra Work. The backup documentation provided contains a math error accounting for the difference in the price presented in the Change Order/Extra Work Order. ISSUE No. 3 -CEI UTILITY POLE SUPPORT FEE. CEI required the Contractor to pay a fee for supporting a utility pole in close proximity to the proposed waterline installation. This condition also creates an addition to the contract value of $4,129.65, which consists of the fee paid ($3,933.00) and the Contractor's 5% markup ($196.65). The backup documentation provided contains a math error accounting for the difference in the final price presented in the Change Order/Extra Work Order.
