The laboratory report shall contain aI[ of the .fallowing: A, The signature, under oath, of any person who pcrfortned the analysis; l3. Any tin@ings as to the identity and quantify of alcohol, a drug of ahttso, or a combination of them that was Found; C. A copy of a notarised statement by the laboratory director or a designee of the director that Contours the ttamc of each certified analyst or iest performer involved with the report, 117c analyst's or test performer's errtployntent relationship with the laboratory that issued the repast, and a notation that performing an analysis of the type involved is pact of the analyst's or tort performer's rcguIar duties; D. ~n outline of the analyst's or test performer's education, training, aril experience in performing the type of analysis involved and a certifteatinn that rite laboratory satisfies appropriate quality control standards in general attd, in this particular aitalysis,~ under rules of the Department Af I.Iealth, (2} Notwithstanding any other provision of law regazding the admission of evidence, a report of the type described in subsection (fl(1) of this section is pat aclmissi6le against the defendant to whottt it pertains in aqy proceeding, other than a preliminary hearing ar a grand jury proceeding, unless the prosecutor has served a copy of the report on the defcnclaat's attorney or, ifthe defendant has no attorney, on the defeud~nt.
