LNDUSTRIAL ADVISORS BUREAU, INC. CLEVELAND WORKERS' COMPENSATION Service Agreement THIS AGREEMENT entered into at Cleveland, Ohio, this day of by and between Village of Mayfield of 6621 Wilson Mills Rd. , Mayfield Village, OH ~ 44143 , 1~ereinafter referred to as EMPLOYER, and INDUSTRIAL ADVISORS BUREAU, INC., of 714 Superior Building, Cleveland, Ohio. INDUSTRIAL ADVISORS BUREAU, INC. hereby agrees to furnish to the Employer a complete service on all matters pertaining to the business of the said Employer which may arise out of their participation as subscribers to the State Insurance Fund, the Industrial Commission of Ohio and the Department of Industrial Relations, as follows: To make a thorough actuarial survey and audit of their rates and classi- fications. • To make a thorough inspection and audit of all charges and claim reserves. To provide consultation service on any and all matters in connection with claims, classifications and rates. To provide consultation service on any and all accounting and actuarial matters which may apply to their participation in the State Insurance Fund. To check and file all reports, claims and settlement sheets in order to insure compliance with the rules and regulations of the Industrial Commission of Ohio. To attend bearings before the Bureau of Worl;ers' Compensation, Industrial Corn- mission of Oliio, and Regional Boards of Review. EMPLOYER hereby agrees to forward all claims, claim information, hearing notices, payroll reports-settlement sheets, rate matters, and all Industrial Commission correspondence to Industrial Advisors Bureau, Inc., and further agrees to keep said Industrial Advisors Bureau, Inc. advised of operational changes and all other matters that may affect its position with the Industrial Commission of Ohio.
