M.V. ORDINANCE N0. 90-14 <br />Page -14- <br />3. Upon retirement, all full-time employees shall be entitled to receive <br />credit and payment for accumulated but unused sick leave not to exceed <br />thirty (30) days provided that. they have been employed by the Village <br />for ten (10) years or more. For purposes of this Section, an employee <br />shall be considered to have retired when such employee takes action <br />that is inconsistent with any reasonable intent to continue employment <br />with the Municipality. <br />4. If a full-time employee is scheduled to work on a holiday enumerated <br />in Section 6(a), and cannot work due to illness, that day shall be <br />considered a holiday day off and shall not be counted or compensated <br />as a sick day. <br />H. Bereavement. Leave <br />In the event of death in the employee's immediate family, he shall be <br />entitled to three (3) days off, or in the case of a member of the Fire <br />Department under. the rank of Chief, one (1) twenty-four (24) hour shift. <br />The "immediate family" shall include the spouse, children, parents, <br />brothers, sisters, spouse's parents and grandparents. Should an exception <br />to the aforementioned apply, such exception to be authorized by the Mayor. <br />This leave shall not be charged to any other leave provision provided by <br />the Ordinance. <br />I. Hospitalization and. Medical Coverage <br />1. The Municipality shall pay the premiums for all permanent full-time <br />employees insured under a contract of hospitalization and medical <br />coverage selected by Mayfield Village. <br />2. Permanent full-time employees of the Municipality not presently <br />insured and new permanent full-time employees of the Municipality <br />may join the plan at such intervals as may be authorized by the <br />Municipality's medical insurance carrier, subject to the approval <br />of the Mayor and Finance Director. <br />3. Any permanent full.-time employee who is otherwise eligible to be <br />covered under a contract of hospitalization and medical coverage but <br />voluntarily withdraws from the above-referenced insurance coverage <br />shall be paid the amount of One Thousand Dollars ($1,000.00) for each <br />year the employee is employed by Mayfield Village but does not request <br />coverage. The payment authorized herein shall be made in equal <br />quarterly installments. <br />
