M.V. ORDINANCE NO. 90- 48 <br />Page 14 <br />I. Hospitalization and Medical Coverage <br />1. The Municipality shall pay the premiums for all permanent full-time <br />employees insured under a contract of hospitalization and medical coverage <br />selected by Mayfield Village. <br />2. Permanent full-time employees of the Municipality not presently insured <br />and new permanent full-time employees of the Municipality may join the plan <br />at such intervals as may be authorized by the Municipality's medical <br />insurance carrier, subject to the approval of the Mayor and Finance <br />Director. <br />3. Any permanent full-time employee who is otherwise eligible to be <br />covered under a contract of hospitalization and medical coverage but <br />voluntarily withdraws from the above-referenced insurance coverage shall be <br />paid the amount of One Thousand Dollars ($1,000.00) for each year the <br />employee is employed by Mayfield Village but does not request coverage. <br />The payment authorized herein shall be made in equal quarterly <br />installments. <br />J. Overtime <br />1. Overtime will be paid in accordance with the Fair Labor Standards Act, <br />as amended. <br />2. All overtime shall be approved in advance by the Mayor unless there <br />exists an emergency that threatens-the health, safety and welfare of the <br />Municipality. <br />K. Travel Allowance and Conference Expense <br />1. Twenty-Four Cents ($.24) per mile is hereby allowed to reimburse <br />employees of the City who are required to make trips on City business in <br />their own automobiles, other than transportation to and from work, to be <br />paid from the Treasury upon approval of the Mayor and the Director of <br />Finance. <br />2. Any elected or appointed Municipal officer, deputy or assistant or an <br />employee of the Municipal Corporation, may attend, at the expense of the <br />Municipal Corporation, any conference or convention relating to municipal <br />affairs if authorized by the Mayor and the Department Head, and if the <br />fiscal officer of the Municipal Corporation certifies that funds are <br />appropriated and available for such purpose. Such person shall be <br />reimbursed for meals and lodging. <br />
