RESOLUTION N0. 90-28 <br />INTRODUCED BY: Mayor Carmen and Council as a Whole <br />A RESOLUTION <br />AUTHORIZING THE MAYOR AND PRESIDENT OF COUNCIL. <br />TO EXECUTE A CONTRACT WITH COMMUNITY MUTUAL_ <br />AND COMMUNITY LIFE FOR HEALTH AND TERM <br />LIFE INSURANCE, RESPECTIVELY, FOR EMPLOYEES <br />OF HAYFIELD VILLAGE, OHIO, FOR THE PERIOD <br />COMMENCING JUNE 1, 1990 AND <br />TERMINATING FEBRUARY 28, 1991 AND <br />DECLARING AN EMERGENCY <br />BE IT RESOLVED BY THE COUNCIL OF HAYFIELD VILLAGE, OHIO, THAT: <br />SECTION 1. The Mayor and President of Council are hereby authorized and <br />directed to execute a contract with Community Mutual and. Community Life for <br />health and. term insurance, respectively, through agent of record, Lawrence <br />Barrett .of DiBart Insurance Agency, Inc.., for employees of the Municipality <br />for the period commencing June 1, 1990 and terminating February 28, 1991. <br />SECTION 2. The aforesaid contract for health. and. term life insurance <br />shall. be in accordance with the proposals received from Community Mutual and <br />Community Life and shall provide premiums not to exceed those rates estab- <br />lished by the carrier on or before June 1, 1990. Such plan is identified as <br />the Community Preferred Health Plan. <br />SECTION 3. The Council finds and determines that all. formal. actions of <br />this Council relating to the adoption of this Resolution have been taken at <br />open meetings of this Council;. and that deliberations of this Council and of <br />its committees,, resulting in such formal action., took place in meetings open <br />to the public, in compliance, with all statutory requirements including the <br />requirements of Section 121.22 of the Ohio. Revised. Code. <br />SECTION 4. This Resolution is hereby declared to be an emergency measure <br />immediately necessary for the preservation. of the health.,. safety and welfare <br />of the residents of Mayfield Village, Ohio, f.or the.. reason that the current <br />policy of health insurance is set to expire and the maintenance of health <br />insurance coverage for municipal employees and their families is a paramount <br />concern and necessary to maintain morale within. the. municipal employment <br />force. It shall, therefore, take effect immediately upon the passage by the <br />
