Issues discussed usually include but <br />may not be limited to: <br />1. Account structure <br />2. Systems installation <br />3. Enrollment kits <br />4. Contract & booklet development <br />5. Administrative supplies <br />6. Employee identification cards <br />IX. Imolementation of Sienature Services <br />Pursuant to discussions with the Finance Director and Health Care Task Force at Mayfield Village, CHAPMAN AND <br />CHAPMAN will be working toward implementing several signature services: <br />A. Customer Service -call in number and access to service by both the employer and employee directly from <br />Chapman and Chapman <br />B. Communications Support -both written and oral fully customized employee communications such as <br />enrollment and educational meetings, 'lunch and learns,' benefit booklets, FAQ sheets, etc. <br />C. Wellness Coordination -this will include the coordination of health fairs, in-house wellness programs <br />(walking programs, largest looser, nutrition programs, smoking cessation programs (not to include the cost <br />of pharmaceuticals), and the like), and the coordination of outside wellness vendors <br />D. Website and Total Compensation Statements -including the Benergy Website as well as the Real Value <br />Statements provided in paper and/or electronic format setup by CHAPMAN AND CHAPMAN in conjunction <br />with Mayfield Village's Human Resource Department <br />E. Basic HR Consultative Services -assistance with HIPAA, COBRA, FMLA, Healthcare Reform, basic handbook <br />reviews, etc. (Complex HR Consulting will require additional costs) <br />
