~. <br />ORDINANCE NO. 2012-15 <br />INTRODUCED BY: Mayor Rinker and Council as a Whole <br />AN EMERGENCY ORDINANCE <br />AUTH®RIZING AN ACaREEIVIENT WITH IVIETLIFE <br />AS AND F®R AN EIVIPL®YEE AD&®, LIFE <br />AN® ®ENTAL PR®GRAM. <br />WHEREAS, the Health Care Task Force and the Finance Committee <br />have recommended and it is the desire of Council to authorize an AD&D, life <br />and dental plan for employees with MetLife. <br />WHEREAS, due to a decrease in employee participation necessitating a <br />minor increase in rates, the previous legislation, Ordinance No. 2012-13, <br />passed by Mayfield Village Council at a Special Meeting on March 5, 2012, is <br />hereby revised to reflect this increase. <br />NOW, THEREFORE, BE IT ORDAINED BY THE COUNCIL OF THE <br />VILLAGE OF HAYFIELD, COUNTY OF CUYAHOGA (the "Entity") AND <br />STATE OF OHIO THAT: <br />SECTION 1. The Mayor and President of Council are hereby and herein <br />authorized and directed to enter into an agreement with MetLife for AD&D and <br />life, effective April 1, 2012, the cost of which shall be $0.287 per thousand <br />dollars of coverage per month; and .for an employee dental program as <br />stipulated therein, at the rate of $30.63 per month per employee for single <br />coverage, $61.95 per month for employee and spouse, $69.32 per month for <br />employed and children, and $104.50 per month for employee, spouse and <br />children. <br />SECTION 2. The Council finds and determines that all formal actions of <br />this Council relating to the adoption of this Ordinance have been taken at open <br />meetings of this Council; and that deliberations of this Council and of its <br />committees, resulting in such formal action, took place in meetings open to the <br />public, incompliance with all statutory requirements including the requirements <br />of Section 121.22 of the Ohio Revised Code. <br />SECTION 3. This Ordinance is hereby declared to be an emergency <br />measure immediately necessary to maintain the employee AD&D, life and <br />dental program for the employees of Mayfield Village, Ohio which is due to <br />expire on April 1, 2012. It shall, therefore, take effect immediately upon the <br />passage by the affirmative vote of not less than five (5) members elected to <br />
