Ad Hoc Service <br />Ad hoc experience reporting and service, if desired, is available on an hourly basis at a rate of $150.00 <br />per hour. Rates are subject to change only under approval of Cuyahoga County. <br />Services may include, but are not limited to: <br />• Union meeting support <br />• Healthcare Committee management <br />• Wellness Coordination <br />• Any other support activities not covered under the basic service model <br />Political Sub-Divisions can contract with the consultant for services under a global fee format based on <br />mutual agreement between the consultant and the Political. Sub-Division. <br />In consideration of the promises and covenants set forth herein, the Consultant hereby promises and <br />agrees to perform the following services as requested by Political Sub-Division: <br />DISCLOSURE: All fees to be collected from Political Sub-Division and paid to Consultant are as <br />described and disclosed herein, unless amended in writing and attached to this agreement.
