ORDINANCE N0.2010-14 <br />INTRODUCED BY: Mayor Rinker and Council as a Whole <br />AN EMERGENCY ORDINANCE <br />AUTHORIZING AN AGREEMENT WITH <br />GUARDIAN LIFE INSURANCE COMPANY OF AMERICA, <br />AS AND FOR AN EMPLOYEE LIFE AND AD&D INSURANCE PROGRAM. <br />WHEREAS, the Village currently provides all full-time employees with a total <br />of $20,000.00 of Life and AD&D insurance coverage through two separate carriers; <br />and <br />WHEREAS, at this time, it is the recommendation of the Village's Finance <br />Director and Finance Committee and it is the desire of Council to authorize a Life and <br />AD&D insurance plan for employees solely with Guardian Life Insurance Company of <br />America. <br />NOW, THEREFORE, BE IT ORDAINED BY THE COUNCIL OF <br />MAYFIELD VILLAGE, OHIO, THAT: <br />SECTION 1. The Mayor is hereby and herein authorized and directed to enter <br />into an agreement with Guardian Life Insurance Company of America pursuant to the <br />terms, conditions and costs as set forth in Exhibit "A" effective May l, 2010 through <br />Apri130, 2011, or longer. The Mayor is also hereby and herein authorized and directed <br />to terminate the current agreement with Anthem for employee Life and AD&D <br />coverage which was previously entered into. <br />SECTION 2. The Council finds and determines that all formal actions of this <br />Council relating to the adoption of this Ordinance have been taken at open ineetings of <br />this Council; and that deliberations of this Council and of its committees, resulting in <br />such fonnal action, took place in meetings open to the public, in compliance with all <br />statutory requirements including the requirements of Section 121.22 of the Ohio <br />Revised Code. <br />SECTION 3. This Ordinance is hereby declared to be an einergency ineasure <br />immediately necessary for the health, safety and welfare of the residents of Mayfield
