ORDINANCE NO.2009-42 INTRODUCED BY: Mayor Rinker and Council as a Whole AN EMERGENCY ORDINANCE AUTHORIZING AN AGREEMENT WITH GUARDIAN AS AND FOR AN EMPLOYEE AD&D, LIFE AND DENTAL PROGRAM. WHEREAS, the Finance Committee has recommended and it is the desire of Council to authorize an AD&D, life and dental plan for employees with Guardian pursuant to the terms set forth in Exhibit "A" attached hereto and incorporated herein by reference. ? NOW, THEREFORE, BE IT ORDAINED BY THE COUNCIL OF THE VILLAGE OF MAYFIELD, COUNTY OF CUYAHOGA (the "Entity") AloTD STATE OF OHIO THAT: SECTION 1. The Mayor is hereby and herein authorized and directed to enter into an agreement with Guardian for AD&D and life pursuant to the terms, conditions and costs as set forth in Exhibit "A" effective September 1, 2009 through September 1, 2010, and for an employee dental program as stipulated therein, at the rate of $36.80 per month per employee for single coverage, $83.81 per month for employee and spouse, $105.85 per month for employee and children, and $152.88 per month for employee, spouse and children. SECTION 2. The Council finds and determines that all formal actions of this Council relating to the adoption of this Ordinance have been taken at open meetings of this Council; and that deliberations of this Council and of its committees, resulting in such formal action, took place in meetings open to the public, in compliance with all statutory requirements including the requirements of Section 121.22 of the Ohio Revised Code. SECTION 3. This Ordinance is hereby declared to be an emergency measure immediately necessary to maintain the employee ADD&D, life and dental program for the employees of Mayfield Village, Ohio which is due to expire on August 31, 2009.
