ORDINANCE NO. 2009-45 <br />INTRODUCED BY: Mayor Rinker and Council as a Whole <br />AN EMERGENCY ORDINANCE <br />AUTHORIZING AND DIRECTING THE FIRE CHIEF <br />TO SUBMIT AN APPLICATION FOR AND THEREAFTER <br />THE MAYOR AND PRESIDENT OF COUNCIL TO ENTER INTO <br />A PARTICIPATING PROVIDER AGREEMENT WITH <br />ANTHEM BLUE CROSS BLUE SHIELD IN COORDINATION <br />WITH THE VILLAGE'S BILLING SERVICES AGREEMENT <br />WITH LIFE FORCE MANAGEMENT, INC. <br />WHEREAS, the Village has determined it to be in its best interest to implement a process to <br />recover the cost for providing emergency services to non-residents in accordance with Chapter 1523; <br />and <br />WHEREAS, the Village entered into a billing services agreement with Life Force <br />Management, Inc. to facilitate the recovery of the costs associated with providing emergency services <br />in accordance with Chapter 1523; and <br />WHEREAS, at this time in coordination with Life Force Management, Inc. the Village <br />wishes to become a participating provider of Anthem Blue Cross Blue Shield; and <br />WHEREAS, as a participating provider of Anthem Blue Cross Blue Shield, the Village will <br />automatically be reimbursed by Anthem at their stated reimbursement rates. <br />NOW, THEREFORE, BE IT ORDAINED BY THE COUNCIL OF MAYFIELD VILLAGE, <br />OHIO, THAT: <br />SECTION 1. The Mayor and President of Council authorize Fire Chief Dave Mohr to <br />submit an application to become a Participating Provider and thereafter the Mayor and President of <br />Council are authorized and directed to enter into a Participating Provider Agreement with Anthem <br />Blue Cross Blue Shield in a form substantially similar to that attached hereto and incorporated herein <br />by reference. as Exhibit "A". <br />SECTION 2. The Council finds and determines that all formal actions of this Council <br />relating to the adoption of this Ordinance have been taken at open meetings of this Council; and that <br />deliberations of this Council and of its coinmittees, resulting in such formal action, took place in <br />meetings open to the public, in compliance with all statutory requirements including the <br />requirements of Section 121.22 of the Ohio Revised Code.
