Mar 14 03 02:39p Diemert & Associates (440)442-0825 p.2 <br />ORDiNANCE NO. 2003- 09 <br />1NTRODUCED BY: Mayor Rinker and Council as a Whole <br />AN EMERGENCY ORDINANCE <br />AUTHORIZING AN AGREEMENT WITH ANTHEM BLUE CROSS <br />AND BLUE SHIELD AS AND FOR THE RENEWAL OF EMPLOYEE HEALTH <br />CARE AND HOSPITALIZATfON BENEFITS, INCLUDING <br />LlFE INSURANCE AND AD&D BENEFITS <br />WHEREAS, the current health care and hospitalization insurance has expired as of <br />April 1, 2003; and <br />WHEREAS, it is the recommendation of the Finance Committee of Council that this <br />legislation be passed to renew empioyee health care and hospitalization benefits, including <br />life insurance and AD&D benefits. <br />NOW, THEREFORE, BE IT ORDAINED BY THE COUNCIL OF MAYFIELD <br />VILLAGE, OHIO, THAT: <br />SECTION 1. That the Mayor and Director of Finance are hereby and herein <br />authorized and directed to enter into a health care and hospitalization policy for ernployees, <br />including life insurance and AD&D benefits of the Vi(lage with Anthem Blue Cross and Blue <br />Shield pursuant to the rates attached hereto and incorporated herein as Exhibit "A". <br />SECTION 2. The Council finds and determines that all formal actions of this Council <br />relating to the adoption ofthis Ordinance have been taken at open meetings of this Council; <br />and that deliberations of this Council and of its committees, resulting in such formal action, <br />took place in meetings open to the public, in compliance with all statutory requirements <br />including the requirements of Section 121,22 of the Ohio Revised Code, <br />SECTION 3. This Ordinance is hereby declared to be an emergency measure <br />immediately necessary for the health, safety and welfare of the residents of Mayfield <br />Village, Ohio, and for the further reasons set forth in the preamble hereof.
