ORDINANCE NO. 2003-35-PAGE 11 <br />153.12 HOSPITALIZATION AND MEDICAL COVERAGE. <br />(a.) The Municipality shall pay a portion of the premiums for all permanent full-time <br />employees insured under a contract of hospitalization and medical coverage selected by Mayfield <br />Village. <br />(b.) Permanent full-time employees of the Municipality not presently insured and new <br />permanent full-time employees of the Municipality may join the plan at such intervals as may be <br />authorized by the Municipality's medical insurance carrier, subject to the approval of the Mayor and <br />Finance Director. <br />(c.) Any permanent full-time employee who is otherwise eligible to be covered under a contract <br />of hospitalization and medical coverage but voluntarily withdraws from the above-referenced <br />insurance coverage shall be paid the amount equivalent to 40% of the premium paid by the Village <br />for the particular coverage for each year the employee is employed by Mayfield Village but does <br />not request coverage (e.g., if the annual premium for medical coverage and dental coverage .is <br />$10,000.00 and $2,000.00, respectively, an employee who waives both hospitalization and dental <br />shall receive $4,000.00 plus $800.00 in lieu of hospitalization and dental coverage for the year in <br />question). The payment authorized herein shall be made in equal quarterly installments. This <br />payment will not be made to any person who is otherwise covered under the Mayfield Village <br />policy through a Village employed spouse or other family member. (Effective in 1994). If an <br />employee is eligible under the terms of this paragraph but has worked less than one full year, the <br />benefit described herein will be prorated on a monthly basis beginning the first month in which the <br />employee would have received the benefit under the plan provided by the Village. <br />(d.) The Village will establish a"Health Care Task Force" comprised of appropriate <br />representatives of the Village administration and Village employees, including representatives of <br />the F.O.P., the Fire Union and representatives from each department. The Task Force will be <br />expected to study the problems of health care and health care cost containment and then formulate <br />recommendations which will be considered by the Village. <br />(e.) The maximum monthly premium expense assumed per employee by the Village for the <br />above medical plans shall be as follows: <br />For the Calendar Year 2003 2004 2005 <br />Employee $340 $364 $389 <br />Employee & Spouse $790 $845 $904 <br />Employee & Children $635 $679 $727 <br />Family $1,140 $1,220 $1,305
