CENTER/WHITE ROAD WIDENING <br />DESIGN AND PLAN PREPARATION DOCUMENTS <br />URS / MAYFIEI.D VILLAGE <br />AGREEMENT DATED FEBRUARY 3,1998 <br />MODIFICA TION No. 2- DATED I)ECEMBER 08, 2000 <br />This Work Order Task is being performed as an extension of the Professional Engineering Services <br />Agreement between Mayfield Village and URS dated February 3, 1998. All terms and conditions stated <br />therein are incorporated into this Work Order by reference. TI:e Mod No. 2 modifacations to the base <br />document are annotated by bold atadics font <br />SCOPE OF SERVICES <br />URS will provide the following specific consulting services to perform the design and plan preparation <br />documents needed for the widening and reconstruction of S.O.M. Center/White Roads. S.O.M. Center <br />Road would be widened to five (5) lanes for an estimated length of twelve thousand seven hundred <br />(12,700) lineal foot. White Road would be widened to three (3) lanes for an estimated length of twelve <br />hundred (1,200) lineal foot. The work would include three (3) signalized intersections. The design and <br />plan preparation services would be performed to be consistent with the new ODOT Local Public Agency <br />Policy. <br />1. Alternative Alignanent Study: URS will prepare an alignment study for S.O.M. Center Road <br />from White Road to Wilson Mills Road. This alignment study will identify potential right of way <br />impacts, potential environmental impacts, and potential utility impacts of providing a five lane <br />roadway section throughout the project limits. URS will prepare a preliminary plan that addresses <br />these concerns and present it to the Village for approval before proceeding with final design. <br />Additional alignment studies, meeting displays, grapl:ics, and `neeting attendance with tlze <br />Vildage's SOM Widening Committee. <br />2. Field Survey: Using conventional methods, URS will perform field surveys consisting of the <br />following elements: <br />• Establish centerlines of S.O.M. Center Road, Wilson Mills, Highland and White Road <br />• Cross Sections for proper drainage and alignment (every 50') <br />o General topo (hydrants, catch basins, water, traffic, sewer, etc.) <br />• Right-of-way delineation <br />• Bench Marks <br />• Reference Points <br />• Aerial Mapping (Wilson Mills to 1800 ft north of Highland ) <br />• Field staking proposed riglat of way per direction of SOM Widei:ing Committee. <br />Bench marks and control points will be established at 800 foot maximum spacing throughout the <br />project. Cross sections will be taken at fifty-foot intervals and 100 feet left and right laterally <br />within the stated limits. Topography will be located within the project limits. All the survey data <br />will be collected using an electronic total station survey system and converted to base mapping in <br />an AutoCAD format. Base plans and profiles will be prepared from the survey data collected and <br />verified. <br />Detailed Design and Plans Preparation: URS will prepare design plans in accordance with <br />ODOT design standards and construction specifications for the widening and reconstruction of <br />S.O.M.
