WORK ORDER N0.3 <br />NORTHWEST QUADRANT PROJECT -SITE UTILITY PLANS <br />URS GREINER /HAYFIELD VILLAGE <br />AGREEMENT DATED FEBRUARY 3,1998 <br />This Work Order Task is being performed as an extension of the Professional Engineering Services <br />Agreement between Mayfield Village and URS Greiner (URSG) dated February 3, 1998. All terms and <br />conditions stated therein are incorporated into this Work Order by reference: <br />SCOPE OF SERVICES <br />URSG will provide the following specific consulting services to prepare construction drawings associated <br />with fifteen hundred lineal foot of new water mains and sanitary collector sewers to serve the "Progressive" <br />site: <br />Utilizing the base topographical mapping already procured by the Client, URSG will prepare plan, <br />profile and detail drawings showing the locations, elevations, arrangements and details associated <br />with the new water main and sanitary sewer. URSG shall also prepare the work/material specification <br />sections required for-the execution and testing of the planned improvements. <br />2. URSG shall complete a Plan Approval and aPermit-to-Install application and other data sheets <br />required by the Ohio EPA and submit the same to the Ohio EPA for review and approval. <br />URSG shall prepare an estimate of the probable construction cost of the improvements reflected in <br />the completed detail plans. <br />ADDITIONAL SERVICES . <br />In addition to the Basic Scope of Services provided under this Work Order, URS Greiner can provide the <br />following services when authorized, in writing, by the Client: <br />1. Perform water system pressure tests that maybe required to confirm available water capacity; <br />2. Perform an analysis of available sanitary sewer capacity at S.O.M. Center Road sewer main. <br />ESTIMATED COST <br />The total work order costs will be based on only the actual labor hours and reimbursable expenses expended <br />on the Project. URS Greiner's compensation for this Work Order will not exceed the authorized amount of <br />Eight Thousand ($8,000.00) Dollars without further written approval from the Client. <br />SCHEDULE OF COMPLETION <br />URSG is prepared to commence work as soon as written authorization to proceed is received from the Client. <br />URSG will complete the services to be provided under this Work Order within four (4) weeks of such <br />authorization contingent upon timely receipt of information and documents from others. <br />Q:~BD~50306Q~1T-VIEW~WKORD3TO.WPD 1 URS Greiner <br />
