Agreement between Mayfield Village and F.O.P. Lodge 57 <br />Section 31.2. The compensation provided herein shall be earned on a monthly basis, with <br />payments made in January and July of every year, for the six months preceding the payment <br />date. <br />Section 31.3. Should an employee terminate his employment with the Village, having accrued a <br />portion of said benefit but not yet having received payment, the prorated amount due the <br />employee will be paid as soon thereafter as may be practical. <br />Section 31.4. All permanent full-time employees shall be eligible for reimbursement of tuition <br />costs, as set forth below, resulting from the employee taking courses from an accredited <br />institution of higher learning, provided that: <br />a) They are reasonably related to the employee's employment with the Village, with such <br />relationship to be determined in the Village's sole discretion; <br />b) The taking of the course has been approved in advance by the Department Head and the <br />Mayor. <br />c) Proof of the grade received and receipt for the tuition are submitted to the Director of <br />Finance within 30 days of completion of the course. <br />d) Any employee who leaves the employ of the Village for other employment within two (2) <br />years after reimbursement of tuition costs shall reimburse the Village for said reimbursed <br />tuition costs. <br />e) In order to be eligible for reimbursement, the employee must obtain a grade of "C" or <br />better in the course or, in the case of a class taken on a"pass/fail" basis, a"pass" must be <br />obtained. <br />f) The amount of tuition reimbursed shall be limited to the amount of the then current per <br />credit hour cost charged by the institution the employee attends, not in any event to <br />exceed the rate charged by Cleveland State University at the time of the employee's <br />reimbursement request. <br />ARTICLE 32 <br />BREATH TEST MACHINE PROFICIENCYALLOWANCE <br />Section 32.1. Annually, and in accordance with standards set forth by the State of Ohio, <br />Department of Health, in the Ohio Revised Code §3701.143, all bargaining unit members who <br />have completed their probationary period and who achieve Breath Test Machine Proficiency <br />certification shall be entitled to a payment of $1,650.00 in each year of the contract. <br />Section 32.2.
