EMPLOYEE BENEFITS INTERNATIONAL EMPLOYEE BENEFITS INTERNATIONAL REGIONAL PARTNER AGREEMENT The following Agreement outlines Employee Benefits International's (EBI) relationship with the Cuyahoga County Regional Partner. This agYeement is made by and between Mayfield Village heYeinafter xeferYed to as "Regional Partner", and Employee Benefits International, hereinafter referred to as "Consultant" on this September 22, 2014. This contract shall run foY thirty six (36) consecutive months commencing on January 1, 2015 and concluding on December 31, 2017. The Regional Partner named herein has agxeed to paYticipate in the Cuyahoga County Sponsored Insurance plans, hereinafter referted to as "Regionalization PYOgram" as outlined in the Health Insutance Guideline document. To participate in the Regionalization Pxogtam, the Regional PaYmer agxees to pay the following applicable fees to the Consultant (Subject to change with prior approval from Cuyahoga County): 1. Underwriting and Renewal Fees Fees are payable for the Initial Underwriting and Renewal of each Regional PaYtnex whether or not they are accepted into the County's Regionalization Program. Fees will be waived if the Regional Partner agYees to join the regionalization: Number of Covered Employees Fee • Less than 100 $1,000 • 100 or more employees $2,000 2. Fees ,• Basic Fee -$8.00 per covered employee peY month • Enhanced Fee -$15.00 per coveYed employee per month • Service Fees are billed monthly or quarterly, with a minimum fee of $500 peY quaxter. Regional PartneYS can maintain existing bYOker relationships on a diYect fee for service basis only. Outside brokers and consultants will not be able to resolve claim or eligibility issues. This responsibility falls to either the County or the County's consultant. Non-Contracted Agencies, Agents, or Brokers must abide by all requirements set forth by the County and no cominissions are payable undeY the Regionalized Program. Othex Benefits (i.e., Dental, Vision, Gxoup Life Insurance, and Voluntary Benefits also known as "Affinity PYOgrams") are optional and not covexed under this agreement. These benefits are coinmissionable, and subject to standard Yegionalized or state-ftled comtnission Yates. Outside bYOkers and consultants cannot access County-sponsored regional affinity progYams.
