<br />ORDINANCE NO. 2015-02 <br />INTRODUCED BY: Mayor Rinker and Council as a Whole <br />AN EMERGENCY ORDINANCE <br />AUTHORIZING AN AGREEMENT WITH DELTA DENTAL <br />AS AND FOR AN EMPLOYEE DENTAL PROGRAM. <br />WHEREAS, the Health Care Task Force and the Finance Committee <br />have recommended and it is the desire of Council to authorize a dental plan for <br />employees with Delta Dental. <br />NOW, THEREFORE, BE IT ORDAINED BY THE COUNCIL OF THE <br />VILLAGE OF HAYFIELD, COUNTY OF CUYAHOGA (the "Entity") AND <br />STATE OF OHIO THAT: <br />SECTION 1. The Mayor and President of Council are hereby and herein <br />authorized and directed to enter into an agreement for a period of two (2) years <br />with Delta Dental effective April 1, 2015, for an employee dental program as <br />stipulated therein, at the rate of $30.99 per month per employee for single <br />coverage, $62.70 per month for employee and spouse, $73.58 per month for <br />employee and children, and $114.85 per month for employee, spouse and <br />children. <br />SECTION 2. The Council finds and determines that all formal actions of <br />this Council relating to the adoption of this Ordinance have been taken at open <br />meetings of this Council; and that deliberations of this Council and of its <br />committees, resulting in such formal action, took place in meetings open to the <br />public, in compliance with all statutory requirements including the requirements <br />of Section 121.22 of the Ohio Revised Code. <br />SECTION 3. This Ordinance is hereby declared to be an emergency <br />measure immediately necessary to maintain the employee dental program for <br />the employees of Mayfield Village, Ohio which is due to expire on April 1, 2015. <br />It shall, therefore, take effect immediately upon the passage by the affirmative <br />
