Exhibit A Description of Services: A. Village acknowledges that the County has the discretion as to which (if any) plan and/or plan design options it may offer to a Village including the offerings within a plan and the County reserves the right to change the plan offerings at its sole discretion. B. The Village acknowledges that the County has the sole discretion as to the fully insured equivalent rate, administrative fees, the risk surcharge and any other fees. By entering into this Contract and/or renewing said Contract, the Village agrees to pay the fully insured equivalent rate, administrative fees, the risk surcharge and any other fees set by the County. C. Village is solely responsible for any and all run out claims that occur prior to joining this program/plan. D. The County shall have a right to conduct an eligibility audit of employees of the Village and participants to determine their eligibility for this program. E. Village agrees to submit only valid claims from eligible participants. If a participant is deemed ineligible, then the Village shall pay back the County any monies expended on that ineligible claim within 30 days. F. Village shall pay all fees, surcharges, rates, etc. on an as billed basis within 10 days of an invoice and such funds are not subject to being refunded by the County and the County shall use such funds to pay its costs; pay for stop loss insurance, establish a pool, build up a reserve, and pay for other items related to health benefits.
