EX)=IIBIT 1 <br />Scone of Work and Project Budget <br />On May 19, 2016, the Local Government Innovation Council approved Mayfield Village's project, for up <br />to $100,000.00 in Local Government Safety Capital Grant Program funding. Grant Funds up to <br />$100,000.00 from fund SRDO, account line item 195666, Local Government Safety Capital Grant <br />Program, for fiscal year 2016 are to be used for eligible costs associated with the Project. <br />Mayfield Village is looking to consolidate services in order to save money along with improving safety. <br />Dispatch consoles are being purchased in order to provide the highest quality of service and public safety <br />to the community. <br />Total project costs are estimated to be $296,596.00. The Mayfield Village is committed to funding the <br />balance of the Project. <br />Sources and uses of funds are as follows: <br />Sources of Funds <br />Local Government Safety Capital Program Grant $100,000.00 <br />Local Match $196,596.00 <br />Total $296,596.00 <br />Uses of Funds <br />MCC 7500 IP Dispatch Console $296,596.00 <br />Total $296,596.00 <br />The final deliverables from the Mayfield Village to the Ohio Development Services Agency will be: <br />(1) The Closeout Report in the form provided to the Grantee by the Ohio Development Services <br />Agency. <br />Local Government Safety Capital Grant Page 11 of 11 <br />
