Customer understands that it does not need to give this consent in order to purchase goods or services from Supplier. Utility: DEO Utility Account Number(s): See Addendum A for account numbers, service and billing addresses. Federal Tax ID Number: Contract Price: $2.61 Per Unit The Contract Price, as set forth above, does not include taxes or Utility charges. "Unit' means Ccf if Columbia Gas of Ohio is the "Utility' searing your account(s), or Mcf if Dominion East Ohio is the "Utilit~l' serving your account(s). This Natural Gas Sales Agreement is comprised of this Base Contract, Addendum A -Account Information and Addendum B -Terms and Conditions (collectively, the "Agreement"). The Agreement is binding only if signed by both ou and Supplier. You are responsible to provide Supplier your current contact information as included in the Customer ection above. If your contact information changes, you must notify Supplier using Supplier's contact information shown above. Addendum A -Account Information and Addendum B- Terms and Conditions, are incorporated herein by reference. Tax Exemption: A completed tax exemption certificate must accompany this Agreement. If no certificate is attached, Customer will remain subject to taxes until a valid certificate is received and processed. Informational Calls: By providing a telephone contact number Customer agrees that Supplier and its service providers may use the number to contact Customer via calls and text messages, including. automatically dialed calls and text messages, and calls that use an artificial or prerecorded voice, regarding any account, billing or service related matter. Authority: You hereby represent to Supplier: (i) the person signing this Agreement on your behalf is legally authorized to do so; (ii) the Agreement, when signed by both you and Supplier, will be valid and legally binding upon you and enforceable in accordance with its respective terms; (iii) you have full power and authority over the provision of natural gas supply to each of the facility service addresses to which the Account(s) pertain; and (iv) the execution, delivery and performance of this Agreement have been duly and validly authorized by all necessary corporate or other actions, which actions shall include, if you are a Ohio county, township, municipal corporation or other Ohio governmental entity, all actions required under Ohio state or local laws and/or regulations. Counterparts and Signatures: This Agreement may be signed in counterparts which, when taken together, will comprise the entire'Agreement. Electronic copies of this Agreement signed manually or electronically may be used for all purposes under which an original agreement bearing original signatures could be used. By signing this Agreement, you hereby agree, as of the Effective Date, that you have read and understand the terms and conditions of this Agreement.
