TITIS <br />WORK ORDER NO. 7 <br />S.O.M. CENTER/WHITE ROAD WIDENING <br />ON-SITE REPRESENTATION DURING CONSTRUCTION <br />URS CORPORATION/MAYFIELD VILLAGE <br />AGREEMENT DATED FEBRUARY 3,1998 <br />MODIFICATION NO.10 - Dated June 2, 2004 <br />This Work Order Task is being performed as an extension of the Professional Engineering Services <br />Agreement between Mayfield Village and URS Corporation (URS), formerly known as URS <br />Greiner, dated February 3, 1998. All terms and conditions stated therein are incorporated into this <br />Work Order by reference. This work order represents the construction observation services <br />previously provided under Work Order No. 8, additional services associated with the MOD NO. 1 <br />modifications due to increasing the length of the S.O.M. Center Road widening to Wilson Mills Road <br />and additional services associated with a longer duration of construction contract and provisions for <br />materials testing. The Mod No. 10 modifications to the base document, which was previously <br />approved by the Client, are annotated by bold italics font. <br />SCOPE OF SERVICES <br />This Work Order provides for resident representa,tion and materials testing during the construction of <br />. the widening and reconstruction of S.O.M. Center/White Roads. S.O.M. Center Road would be <br />widened to five (5) lanes for an estimated length of twelve thousand seven hundred (12,700) lineal <br />foot. White Road would be widened to three (3) lanes for an estimated length of twelve hundred <br />(1,200) lineal foot. The design, plan preparation, and construction phase services would be <br />performed to be consistent with the new ODOT Local Public Agency Policy. <br />URS will provide the following specific consulting services in assisting the Client during the <br />construction phases of the project: - <br />Construction Phase - Inspection and Observation Services: URS shall provide during the active <br />Construction Phase ihree (3) full-time equivalent inspector(s) for a period not to exceed twenty-four <br />(24) months to endeavor to provide further protection for the Client against defects and deficiencies <br />in the Work. The furnishing of such observation shall not make URS responsible for construction <br />means, methods, techniques, sequences, or procedures, or for the Contractor's failure to perform the <br />Work in accordance with the Contract Documents. The task includes the following: <br />1. URS' inspector shall assist in determining the quantities and dollar amounts owing to the <br />Contractor, based on-site observations and on evaluations of the Contractor's Applications for <br />Payment. The accuracy of each Application for Payment shall remain the primary <br />responsibility of the Contractor. <br />2. URS inspector shall attend job meetings held with the Contractor to coordinate the Work and <br />discuss problems that may arise concerning proper timing and execution of the Work.
