Brett <br />Director of Finance and Economic Development <br />Mayfield Village <br />6621 Wilson Mills Road <br />Mayfield Village, Ohio 44143 <br />Subject: Modification Request for Professional Services Agreement <br />for the SOM Center Road Widening - Modification No.10 <br />Dear Mr. Brett: <br />Please find enclosed herewith, two (2) signed originals of our proposed modifications to our <br />professional engineering services agreement for the Widening of SOM Center Road. The Modification <br />No. 10 revisions to our existing agreement dated February 6, 1998 reflect additional services to be <br />performed in association with construction of the SOM Center Road Widening. <br />The specific revisions to each respective Work Order previously approved and the new work being <br />proposed are as follows: <br />? Work Order No. 6- Construction Administration and Engineering; The scope of work is' <br />being revised to reflect plan changes associated with the 120" junction chamber, 48" wye <br />connection, the Metroparks wall, upper 40 acres wall, extensive R/W and utility coordination, the <br />Metroparks drive relocation, underpass lighting and landscaping. The total Work Order <br />compensation is being increased from $166,600 to $310,000. <br />? Work Order No. 7- Construction Observation; The Scope of Work is being revised to reflect <br />additional staff for construction observation. The total Work Order compensation is being <br />increased from $385,500 .to $514,400. <br />Modification No. 10 reflects a total increase to the base contract in the amount of $272,300. Please <br />review the enclosed documents and call me should you have any questions or wish to meet to discuss <br />them in greater detail. <br />If found to be acceptable, please forwazd one (1) executed,original of the enclosed Work Orders No. 6 <br />and 7 when approved by the Village.
