EXHIBIT A Agreement Givic Cenfer Space Assessmenf Aaree€nent ? MayfieId ViElragc-, Ohio This Agreement, made on (date) between the Mayfield Village, Ohio, (Owner), and Brandstetter Carroll, Inc. (Consultant), whereby the Consultant proposes to provide Planning, Programming and Design Services with respect to the Mayfield Village Civic Center at 6622 Wilson Milis Road. The Consultant proposes to provide the following services: 1. Scope of Services A. Needs Assessment: The Architect will: 1. Review existing layout and use of space. ? 2. Meet with department heads/stakeholders to discuss current and future operations. B. Planning and Design: The Architect will: 1. Prepare floor pian alternatives for.space utilization. 2. Prepare itemized list of ADA upgrades for each alternafiive. ,.3. , Prepare opinion:of-probable cost for each alternative. 4: ; Review;`meet,ings with the Owner as required. C. Final.Presentation / Deliverables: The Architect will: 1. Prepare Final Plan alternative. 2. Prepare presentation documents. 3. Prepare Recommendations Report. 4. Presentations to Owner as required. II. Fees, Conditions and Deliverables A. The Consultant will receive a fee of Nineteen Thousand Four Hundred dollars ($19,400). Billing and payment will be monthly as the work progresses. Upon submission of the first draft report, 90% of the fee will be due, with the 10% balance being due upon submission of the final report (15 copies). The fee includes printing, travel and miscellaneous out-of-pocket expenses. B. The Owner will be responsible to organize and publicize any public meetings, stakeholder meetings, or user group meetings. C. The study will require 16-24 weeks from the commencement of services to the submission of the first draft. The final report will be presented to the Owner within 10 days of receipt of the Owner's final comments.
