MayfieId ViElragc-, Ohio <br />This Agreement, made on (date) between the Mayfield Village, Ohio, <br />(Owner), and Brandstetter Carroll, Inc. (Consultant), whereby the Consultant proposes to <br />provide Planning, Programming and Design Services with respect to the Mayfield Village <br />Civic Center at 6622 Wilson Milis Road. <br />The Consultant proposes to provide the following services: <br />1. Scope of Services <br />A. Needs Assessment: The Architect will: <br />1. Review existing layout and use of space. <br />? 2. Meet with department heads/stakeholders to discuss current and future <br />operations. <br />B. Planning and Design: The Architect will: <br />1. Prepare floor pian alternatives for.space utilization. <br />2. Prepare itemized list of ADA upgrades for each alternafiive. <br />,.3. , Prepare opinion:of-probable cost for each alternative. <br />4: ; Review;`meet,ings with the Owner as required. <br />C. Final.Presentation / Deliverables: The Architect will: <br />1. Prepare Final Plan alternative. <br />2. Prepare presentation documents. <br />3. Prepare Recommendations Report. <br />4. Presentations to Owner as required. <br />II. Fees, Conditions and Deliverables <br />A. The Consultant will receive a fee of Nineteen Thousand Four Hundred dollars <br />($19,400). Billing and payment will be monthly as the work progresses. Upon <br />submission of the first draft report, 90% of the fee will be due, with the 10% <br />balance being due upon submission of the final report (15 copies). The fee <br />includes printing, travel and miscellaneous out-of-pocket expenses. <br />B. The Owner will be responsible to organize and publicize any public meetings, <br />stakeholder meetings, or user group meetings. <br />C. The study will require 16-24 weeks from the commencement of services to the <br />submission of the first draft. The final report will be presented to the Owner within <br />10 days of receipt of the Owner's final comments.
