j RESOLUTION 2023-58 INTRODUCED BY: Mayor Bodnar AN EMERGENCY RESOLUTION AUTHORIZING AN AGREEMENT WITH VSP FOR AN EMPLOYEE VISION CARE PLAN WHEREAS, the Director of Finance has recommended, and it is the desire of Council to authorize the renewal of the vision care plan ("Plan") for employees with VSP; and WHEREAS, the Plan would automatically renew on January 1, 2024 at the additional rate of .02 per month with renewal rates guaranteed for two (2) years. NOW, THEREFORE, BE IT RESOLVED by the Council of Mayfield Village, Cuyahoga County, State of Ohio that: SECTION 1. The Mayor and President of Council are hereby and herein authorized and directed to enter into an agreement for a period of forty-eight (48) months, commencing on January 1, 2024 and concluding on December 31, 2025 with VSP for an employee vision care program as stipulated therein, at the rate of $8.06 per month per employee for single coverage, $13.57 per month for employee and spouse, $13.85 per month for employee and children, and $22.34 per month for employee, spouse and children, all as is more fully set forth in Exhibit "A", a copy of which is attached hereto and incorporated herein. SECTION 2. The Council finds and determines that all formal actions of this Council relating to the adoption of this Resolution have been taken at open meetings of this Council; and that deliberations of this Council and of its committees, resulting in such formal action, took place in meetings open to the public, in compliance with all statutory requirements including requirements of Section 121.22 of the Ohio Revised Code.
