The primary reason for his visit at the University was to verify his <br />visual status in applying for the North Olmsted Fire Department. On October 29"' Timothy was <br />measured with a distance visual acuity corrected with contact lenses at OD 20/25-2 and OS 20/25+. <br />When both eyes were used together (binocular) his vision was found to be 20/20. Timothy's stereopsis <br />was found to be 40 seconds of are on a ray and dot test. <br />In surnrnary, Asir. Gates does meet the requirements set forth by the North Olmsted Fire Department <br />as pleasured by two other independent sources. Those two sources are the Ohio State University and <br />Lakeland Eye Surgeons & Associates. Since Mr. Gates was able to pass these requirements at two <br />other offices, my final recommendation would be to accept him as an applicant. I would recommend <br />that you stipulate that he obtain a yearly eye examination in order to prove his contact lens wearing <br />ability and that he continues to meet the visual acuity requirements. <br />I hope that this letter is helpful to you in making a final decision.
