APPLICATION INSTRUCTIONS: <br />IT IS IMPORTANT THAT EACH CANDIDATE READ ALL THE APPLICATION <br />INSTRUCTIONS PRIOR TO TURNING IN APPLICATION! <br />(Failure to follow any or part of the enclosed instructions will result in the rejection of your application). <br />Only one application per person may be submitted. <br />1. All applicants shall be an Ohio State Certified paramedic to be eligible to participate in the written <br />examination. The required certification must be in accordance with the level of qualifications mandated <br />by the North Olmsted Fire Department. Applicants shall include a copy of his/hers valid/current Ohio <br />State Paramedic card in the application packet. Applicants shall have maintained appropriate continuing <br />education training hours as mandated by the State of Ohio for paramedic recertification. Applicants <br />shall present a copy of his/hers continuing education training hours to the Fire Chief during the <br />interview process. Any required continuing education training hours not current at date of appointment <br />shall be completed by the applicant at his/her expense. <br />2. All applications must be picked up at the NORTH OLMSTED CITY ADMINISTRATION <br />BUILDING, 5200 DOVER CENTER ROAD, NORTH OLMSTED, OHIO 44070, between the hours <br />of 9:00 A.M. and 4:30 P.M. Monday through Friday. Only the first 200 candidates who submit properly <br />completed applications will be allowed to sit for the written test. <br />3. All applications must be legibly printed in ink or typed, contain a full return address including zip code; <br />and be signed and dated by the applicant. Applicants must also list their name and address, including zip <br />code, on the front of the return envelope provided. HAND DELIVER COMPLETED PACKETS TO <br />North Olmsted Police Department Attn. Applicants for Entry -Level appointment to the Fire Department as a Firefighter/Paramedic shall satisfy the age <br />requirements and physical qualifications established by Ohio Revised Code Section 124.42 to be eligible <br />for appointment. (No person shall be eligible to receive an original appointment as a <br />Firefighter/Paramedic in a fire department, subject to the civil service laws of this state, unless the <br />person has reached the age of eighteen and has, not more than one hundred twenty days prior to <br />receiving such appointment, passed a physical examination, given by a licensed physician, a physician <br />assistant, a clinical nurse specialist, a certified nurse practitioner, or a certified nurse -midwife, certifying <br />that the applicant is free of cardiovascular and pulmonary diseases, and showing that the person meets <br />the physical requirements necessary to perform the duties of a Firefighter/Paramedic as established by <br />the civil service commission having jurisdiction over the appointment. The appointing authority shall, <br />prior to making any such appointment, file with the Ohio police and fire pension fund a copy of the <br />report or findings of said licensed physician, physician assistant, clinical nurse specialist, certified nurse <br />practitioner, or certified nurse -midwife. The professional fee for such physical examination shall be paid <br />by the civil service commission. No person shall be eligible to receive an original appointment on and <br />after the person's thirty-first birthday). <br />5.
