Ohio R.evised Code Secfion 3517.13(j)(3) Certification <br />(For Oluo corporations) <br />The undersigned officer of Richard L. Bowen & Assaciates, Inc., an Ohio for profit <br />corporation, acknowledges the provisibns of Section 3517.13 of the Ohio Revised. Code, <br />which establish eerhain cainpaign contribution limits and requirements for contkactors that <br />are awazded municipal contracts for goods or services with a cost aggregating to more <br />than ten thousand dollars ($10,000.(}0) in a calendaz year. <br />The undersigned certifies that all of the following persons related to- the corporation, as <br />applicable, are in compliance wi#h division (J) (1) of saad Section 3517.13: <br />(a) Each owner of rnore than twenty percent of Richard L. Bowen & <br />Associates, Inc; <br />(b) Bach spouse of an owner of mQre than twenty pereent af Richard L. <br />Bowen & Associates, Inc; <br />(c) Each child seven years of age to seventeen years of age of an owner of <br />more than twenty percent of Richard L. Bowen & Associates, Inc.; and <br />(d) Any combinat'ron of persons identified 'zn paragraphs (a) througkr (c), <br />above. <br />Note: Whoever knowingly makes a false statement an a certification reqaired by division (.n <br />(3) of Section 3517.13 of the Ohio Revised Code !s guilty of a felony of the fifth degree, and <br />aay contract that includes, or is dependent upon, the certification made in violation of that <br />divisiun sha11 be rescinded. <br />RICHARU L.
