.? <br />Exhibit 1 <br />Scope of Work <br />The Independent Contractor shall conduct a safety belt education program to third grade students in <br />performing services under this agreement. <br />2. The Independent Contractor will conduct a safety belt educational program for third grade students <br />in any public, private, parochial, and in-home schools within its jurisdiction, using curriculum and <br />informational materials provided by the ODPS. The program will use the formal classroom setting <br />in school facilities during school hours. A classroom schedule will be forwarded to ODPS prior to <br />any presentations to the schools. <br />3. Upon completion of the classroom program, the Independent Contractor shall obtain from an <br />authorized school official (classroom teacher or principal) the completed safety belt education <br />evaluation form(s) for each classroom presentation. The completed evaluation forms must <br />accompany invoices submitted to the ODPS. Under this agreement, the Independent Contractor <br />will be reimbursed $4.00 for each third grader receiving instruction, not to exceed the amount <br />identified on Page 2, Section 3. All instruction must be completed by May 1, 2008. <br />4. All new Independent Contractors participating in the Third Grade Safety Belt Program shall <br />provide ODPS with its formal written safety belt policy, which requires all employees to use safety <br />belts while operating agency vehicles or personal vehicles when engaged in work activity for the <br />contracting agency. The Independent Contractor will conduct a safety belt education program for <br />its employees, to inform them of the benefits of safety belt usage. A statement shall be submitted to <br />ODPS to verify that departmental employees have received safety belt education prior to <br />forwarding the first invoice for payment. <br />The Independent Contractor may not accept any fees from any source other than ODPS for services <br />conducted under this Agreennent. The purchase of any type of equipment to implement this <br />agreement is not authorized. <br />6. For the purpose of this project, the Independent Contractor shall be responsible to the <br />ODPS/Governor's Highway Safety Office (GHSO) Administrator or the assigned designee. The <br />Independent Contractor acknowledges that a11 reports shall be submitted to the GHSO <br />Administrator or the assigned designee and the directions for implementation of the defined activity <br />shall be received from this authority. <br />7. The ODPS will provide the curriculum materials, program evaluation forms, safety belt program <br />literature and other educational materials to the Independent Contractor. ODPS will provide <br />technical assistance to the Independent Contractor to implement this project. <br />8. The ODPS shall not reimburse the Independent Contractor far any travel expenses under this <br />agreement. <br />9. For purposes of this agreement, the condition under Paragraph 2B of this agreement providing that <br />Independent Contractor "shall furnish professional services performed according to applicable <br />commercial standards" means the Independent Contractor shall perform completely and accurately <br />the work and activities described in this "Scope of Wark", as well as meet the requirements under <br />Paragraphs 1-4 above. <br />7of8 <br />
