<br />MEMORANDUM OF UNDERSTANDING <br />BETWEEN <br />THE CUYAHOGA COUNTY <br />BOARD OF HEALTH <br />AND THE <br />CITY OF NORTH OLMSTED <br />THIS MEMORANDUM is entered into thisal8 day of 2007 between the <br />Cuyahoga County Board of Health (hereinafter Board) and the City ofNorthl imsted (hereinafter City). <br />WHEREAS, the Board wishes to enter into a contract with Clarke Environmental Mosquito <br />Management for the (arviciding application of Bradley Woods, and; <br />WHEREAS, a portion of Bradley Woods is located within the City of North Olmsted, and; <br />WHEREAS this application will benefit the residents of the City of North Olmsted, and; <br />WHEREAS the Board and the City agree that in consideration of the Board issuing an RFP for <br />the larviciding of Bradley Woods that the City will pay a portion of the total cost of the application. <br />NOW, THEREFORE, the parties mutually agree as follows: <br />I. TERM. <br />This Agreement is effective for the period April 1, 2007 through May 31, 2007. <br />II. COST. <br />One helicopter larviciding, application will be made to 931.5 Acres at $50.00 per acre with Biosid. <br />Acres Allocated to Each Community <br />Lorain Countv 43.5 Acres <br />North Olmsted 423.5 Acres <br />Westlake 464.5 Acres <br />Total: 931.5 Acres x $50.00/Acre= $46,575.00 <br />III. PROJECT SUMMARY. <br />Clarke will perform a helicopter survey to determine the area of Bradley Woods conducive to <br />larviciding. A map will be made by Clarke showing the area to be sprayed including municipal <br />boundaries and acreage of each community involved. A reproduction of the map will be provided to the <br />Cuyahoga County Board of Health for verification of the spray area. <br />The application will be BIOSID which is an environmentally sensitive formulation consisting of <br />the active ingredient. of inethoprene (0.1 %). Biosid is an insect growth regu(ator. Biosid granules will <br />provide a 14-day residual treatment. <br />M,"
