New Fire Station No. 2 Page 3 of 6 <br />IV. PROPOSAL DEADLINE <br />Three 3-ring presentation binders of a proposal must be received by the City of <br />North Olmsted, 5200 Dover Center Road, North Olmsted, Ohio 44070, no tater <br />than 4:00 p.m., (Enter Day), (Enter Date), 2005. All proposals must be addressed <br />to the attention of Mr. Dan Driscoll, Director of Public Service. All proposals shall <br />be plainly marked on the outside "ARCHITECTURAL SERVICES PROPOSAL". <br />V. ARCHITECTURAL SCOPE OF SERVICES <br />The successful Proposer will provide an architectural scope of services as outlined <br />in APPENDIX B to this RFP. The services will be provided under the Agreement <br />between Owner and successful Proposer. <br />VI. CONTENTS OF PROPOSALS <br />Statements of qualifications should include information regarding: <br />A. Firms history; <br />B. Experience in providing architectural services for projects similar to and <br />larger than the NEW FIRE STATION NO. 2; <br />C. Education and experience of staff who will be involved in the Project; <br />D. Services offered by your firm such as: studies, evaluation, conceptual design, <br />preliminary design, detailed design, production of construction docuxnents <br />(pre-construction, construction and post-construction), and cost estimating; <br />E. Firm's general liability insurance and financial responsibility; <br />F. List any pending or current legal proceedings such as mediation, arbitration <br />or suits; <br />G. References; <br />H. Other services offered by the firm; and <br />1. Documentation indicating firm's compliance or non-compliance with each of <br />the standards listed in Appendix D. <br />J. The Proposer shall include an estimate of his proposed fees including <br />expenses for the project. Since the City uses a qualification-based <br />evaluation system5 this estimat€ shall be limited to one or two pages titat. <br />may be removed from the proposal documents during the evaluation <br />process. <br />Firms who wish to submit a proposal should follow the form of this Request for <br />Proposal. Please refer to Section VII - Subsection F for the specific requirements <br />for which the Proposer should use to document its qualifications since the specific
