PROCEDURES FOR PROVISION OF EQUIPMENT FACILITIES AND/OR SERVICES: <br />a. Requests for equipment, facilities and/or training, either written, oral, or by e-mail, may be addressed to <br />Ms. Lori L. Tedrow (812-854-5641; email: tedrow_lori@crane.navy.mil) or Mr. Darin S. Holder <br />(812-854-5727; email: holder_ds@crane.navy.mil). Written requests should be addressed as foIlows: <br />Commander <br />Code 805E, Bldg. 3291 <br />NAVSURFWARCENDIV <br />300 Highway 361 <br />Crane, IN 47522 <br />b. Upon approval of the request for the loan of equipment, an authorized official of the law enforcement agency shall <br />execute a NAVSURFWARCENDIV Crane Equipment Cusiody Document. The custo@ial document shall include the <br />date of receipt and expected return clate for the equipment, the name of the official signing out and returning the <br />equipment, the office telephone number of the official, agreement number, equipment serial number, and amount of <br />funding received. <br />c. Payment for the provision of equipment and/or services shall be made by check to : <br />Commander <br />Code 805E, Bldg. 3291 <br />NAVSURFWARCENDIV <br />300 Highway 361 <br />Crane, IN 47522 <br />d. Equipment provided under this agreement may be repaired/repIaced by NAVSURFWARCENDIV Crane at its <br />discretion, inclusive of assessment of any costs, during the term of the agreement if failure of operation is caused by <br />other than normal use. Requests for same may be made to Ms. Tedrow or Mr. Holder. All written requests should <br />be addressed as follows: <br />Commander <br />Code 805E, Bldg 3291, L. Tedrow <br />NAVSURFWARCENDIV <br />300 HWY 361 <br />Crane, IN 47522 <br />In replacement scenarios, all transactions will be documented in the Equipment Custody Document indicating a serial <br />number for serial number exchange. Consideration for exchanges of equipment that fails to perform during normal <br />use is included in the agreement fee and is therefore not subject to additional costs. State/local law enforcement <br />agencies will not receive consideration or extension for any period of time during the agreement that equipment <br />should fail or become inoperable. <br />e. The receipt, transporta6on and return of all equipment is the sole responsibility of the requesting state/local law <br />enforcement agency who shall designate in writing a representative authorized to receive, transport equipment to and <br />from NAVSURFWARCENDIV Crane, and return same.
