AGREEMENT <br />between <br />CITY OF NORTH OLMSTED, OHIO <br />and <br />JERRY LUDVIIK <br />for <br />GOLF PRO SERVICES <br />at <br />SPRINGVALE GOLF COURSE <br />THIS AGREEMENT made at North Olmsted, Ohio, on the day of <br />2004 by and between the City of NORTH OLMSTED, 5200 Dover <br />Center Road, North Olmsted, Ohio 44070 [hereinafter referred to as "CITY"], and <br />7ERRY LUDVIIK [hereinafter referred to as "CONTRACTOR"]; <br />WHEREAS, on or about November 26, 1999, the City of North Olmsted entered <br />into an Agreement [hereinafter, the "CONTRACT"] with 7erry Ludvik to provide Golf <br />Pro services at the City's Springvale Golf Course [hereinafter referred to as the <br />"SERVICES"]; and <br />WHEREAS, the CITY'S Council, pursuant to Ordinance Nos. 2000-162, 2001-45 <br />and 2004- , authorized the Mayor to renew said CONTRACT for the SERVICES for <br />the periods commencing April 1 and ending December 31 for the years 2000, 2001, 2002, <br />2003 and 2004; and <br />WHEREAS, the CITY has determined that CONTRACTOR is the individual most <br />qualified to provide the CITY with the SERVICES needed at a price which is fair and <br />reasonable to the CITY, and the Council, pursuant to Ordinance No. 2004-_ has <br />authorized the Mayor to enter into a new Agreement with CONTRACTOR. <br />
