Employees participating and subscribing to the optional health benefits listed above within this <br />section, shall contribute an amount equal to that which is listed within Section 12.2 of this <br />Article. Employees must participate in the medical plan to be permitted to participate within the <br />optiona.l benefit caverage listed within this section. The employee contribution shall be withheld <br />as a payroll deduction not later than the first pay period of each month. <br />12.4 Expected cost or premium is defined as the cost of all medical, hospital, <br />prescription drug, and related fees including, but not limited to administrative fees. <br />12.5 The employer reserves the right to establish all plans, enrollment periods and <br />regulations. <br />12.6 The employer shall make an earnest effort to establish a committee consisting of <br />one (1) representative from each of the city bargaining units, if they choose to be represented and <br />a minimum of one (1) representative of the employer. The duty position. If the employees <br />refizses the position, the use of authorized line of duty sick leave would terminate; and the <br />emplayee wauld have to use their accumulated sick leave. <br />12.7. In 2004 at the conclusion of the bargaining units' current collective bargaining cycle, <br />or sooner if all City bargaining units agree, an employee committee shall be formed and shall meet <br />for the purpase of creating a new or revised health caxe plan or plans for City employees. The <br />comrnittee shall consist of 14 employees who must be participants in one of the City's current <br />health care plans and who have been selected from the following employee groups: 1 frorrd <br />Corrections, 1 from Dispatch, 2 from ClericaUTechnical, 2 from Service, 2 from Fire, 2 from <br />Police, 2 from NOMBL, and 2 from non-bargaining unit employees. The employer shall provide <br />two advisors or facilitators to assist the employee committee regarding health care issues. <br />18
