PROPOSAL DEADLINE <br />Six (6) bound copies of a proposal must be received by the City of North <br />Olmsted, 5200 Dover Center Road, North Olmsted, Ohio 44070, no later than <br />42:00 noon, (Month/day/year). All proposals must be addressed to the attention of <br />Mr. Dan Driscoll, Director of Public Service. All proposals shall be plainly marlced <br />on the outside "ART COORDINATION AND PROCUREMENT SERVICES <br />PROPOSAL". <br />V. ART COORDINATION AND PROCUREMENT SCOPE OF SERVICES <br />The successful Proposer will provide art coordination and procurement scope of <br />services as outlined in APPENDIX B to this RFP. The services will be provided <br />Luider the Agreement between Owner and successful Proposer. <br />VL CONTENTS OF PROPOSALS <br />Statements of qualifications should include information regarding: <br />A. Firms history; <br />B. Experience in providing a.rt coordination and procurenlent services for <br />projects similar to and larger than the North Olmsted Branch Library; <br />C. Qualifications, education and experience of staff wha will be involved in the <br />Proj ect; <br />D. Firm's general liability insurance and financial responsibility; <br />E. List any pendind or current legal proceedings such as mediation, arbitration <br />or suits; <br />F. References; <br />G. Other services offered by the firm; and <br />H. A financial schedule of the Project showing cost for the individual tasks, <br />procurement of the artwork and the compensation of the Proposer. Included <br />in this item will be an estimated total cost for the Project. <br />Firms who wish to subinit a proposal should follow the forrn of this Request for <br />Proposal. Please refer to Section VII - Subsection F for the specific requirements <br />for which the Proposer should use to document its qualifications since the specific <br />requireinents will be used by the City of North Olmsted as the selection criteria in <br />evaluatinD the received Proposals. Please refer to Appendix B for the duties of the <br />successful Proposer. <br />i, _
