<br />by WRAAA in accordance with VVRAAA's Cost Sharing Policy. Contractor shall report aII client <br />Cost Sharing Payments on the Title ItI Quarterly Report and Request for Payment (Quarterly <br />Repor t). Cost Sharing Payments collected from Title III services will be used exclusively to pay for <br />the cost of the Title III service from which it was generated. A failure to collect sums due shall <br />make the Contractor itself liable for the amaunts due. <br />16. Probation. In accordance with Agency Policy 5.9, the Agency may designate a Title III <br />service pxovider with problematic programs as described in said policy as a conditional Contractor <br />and, subject it to a period of probationary status. In event of said designation, the terms of the <br />probation sha11 become an addendum to this contract. <br />17. C.nnfirientialtt-? ncl Disc1nsure nf Tnfnrmatinn. In accord with the Older Americans Act <br />of 1965 as amended, the applicable provisions of tl-ie Code af Federa.l Regulations, the applicable <br />Poiicies and Procedures of the Qhio Department of Aging and the applicable Policies and <br />Procedures of the Agency, the Contractor shall maintain procedures to ensure the protection of the <br />confidentiality of information about older persons coliected in the delivery of services. <br />1$. Tnciir.a„eP. T'he Contractor shall secure and ma.intain at least the following minimum <br />amounts of insurance for the period of this agreement: <br />A. General Iiability insurance, both pnblic and automohile, against claims for <br />injury andlor death in the amaunt of $1,000,000 aggregate and per <br />occurrence. <br />B. Automobile (if transportation is part of the Title III service under this <br />Agreement) and ofher property damage insurance in an amount not less than <br />$50,000 for damages in any one accident or occurrence. <br />C. Fidelity bond or empioyee theft coverage on persons handling Title III program <br />pps 10 of 14 <br />
