CITY OF NORTH OLMSTED RESOLUTION NO. 2002 - 204 BY: Mayor Musial. A RESOLUTION AUTHORIZING THE MAYOR OF THE CITY OF NORTH OLMSTED TO ENTER INTD A TWO-YEAR CONTRACT WITH UNION EYE CARE CENTER, ING, TO PROVIDE EYE CARE AND VISION SERVICES TO CITY EMPLOYEES IN CONNECTION WITH THE CITY'S SELF-INSURED HEALTH CARE PROGRAM FOR CITY EMPLOYEES, AND DECLARING AN EMERGENCY. WHEREAS, it is the desire of this Council, based upon the recommendation of the Director of Personnel and Administrative Services, to authorize the Mayor to enter into a two-year contract with Union Eye Care Center, Inc., to provide eye care and vision services to City Employees in connection with the City's Self-Insured Heaith Care Program for City Employees; and WHEREAS, the proposed contract is a personal services contract for wluch competitive bidding is not required under the City Charter; NOW THEREFORE, BE IT RESOLVED BY THE COUNCIL OF THE CITY OF NORTH OLMSTED, CUYAHOGA COUNTY, AND STATE OF OHIO: SECTION 1: That the Mayor be and he is hereby authorized, subject to approval by the Board of Control, to enter into a contract with Union Eye Care Vision to provide eye care and vision services to City employees in connection with the City's self- insurance plan for City employees. The contract shall be substantially in the form of the "City of North Olmsted Vision Care Agreement," attached to this Resolution and marked as "Exhibit A." SECTION 2: That the contract shall be for a two-year term effective from January 1, 2003 through December 31, 2004. SECTION 3: That this Resolution is hereby declared to be an emergency measure immediately necessary for the preservation of the public health, safety and welfare, and for the reason that it is necessary that this Resolution go into effect immediately so that there is no gap in eye care and vision insurance coverage for City employees as of January 1, 2003, and further provided it receives the affirmative vote of two-thirds of all members of Council, it shali take effect and be in force immediately upon its passage and approval by the Mayor.
