<br />CITY OF NORTH OLMSTED <br />RESOLUTION NO. 2002 - 204 <br />BY: Mayor Musial. <br />A RESOLUTION AUTHORIZING THE MAYOR OF THE CITY OF <br />NORTH OLMSTED TO ENTER INTD A TWO-YEAR CONTRACT <br />WITH UNION EYE CARE CENTER, ING, TO PROVIDE EYE CARE <br />AND VISION SERVICES TO CITY EMPLOYEES IN CONNECTION <br />WITH THE CITY'S SELF-INSURED HEALTH CARE PROGRAM <br />FOR CITY EMPLOYEES, AND DECLARING AN EMERGENCY. <br />WHEREAS, it is the desire of this Council, based upon the recommendation of <br />the Director of Personnel and Administrative Services, to authorize the Mayor to enter <br />into a two-year contract with Union Eye Care Center, Inc., to provide eye care and vision <br />services to City Employees in connection with the City's Self-Insured Heaith Care <br />Program for City Employees; and <br />WHEREAS, the proposed contract is a personal services contract for wluch <br />competitive bidding is not required under the City Charter; <br />NOW THEREFORE, BE IT RESOLVED BY THE COUNCIL OF THE CITY <br />OF NORTH OLMSTED, CUYAHOGA COUNTY, AND STATE OF OHIO: <br />SECTION 1: That the Mayor be and he is hereby authorized, subject to approval <br />by the Board of Control, to enter into a contract with Union Eye Care Vision to provide <br />eye care and vision services to City employees in connection with the City's self- <br />insurance plan for City employees. The contract shall be substantially in the form of the <br />"City of North Olmsted Vision Care Agreement," attached to this Resolution and marked <br />as "Exhibit A." <br />SECTION 2: That the contract shall be for a two-year term effective from <br />January 1, 2003 through December 31, 2004. <br />SECTION 3: That this Resolution is hereby declared to be an emergency measure <br />immediately necessary for the preservation of the public health, safety and welfare, and <br />for the reason that it is necessary that this Resolution go into effect immediately so that <br />there is no gap in eye care and vision insurance coverage for City employees as of <br />January 1, 2003, and further provided it receives the affirmative vote of two-thirds of all <br />members of Council, it shali take effect and be in force immediately upon its passage and <br />approval by the Mayor. <br />
