APPEND(X E <br />(NAME OF PRO)ECT) <br />Additional Criteria for Determining the <br />Responsibility of the Proposer <br />In addition to the primary "responsibilaty qualifications" information requested in the <br />text af Part IV. F of this Request for Proposal (RFP), each Proposer shall furnish the <br />following infornlation regardi.ng the following items. <br />1. Documentation from previous projects regarding timeliness of performance, <br />quality of work, extension requests, fines and penalties imposed and payment <br />thereof? liens filed, history of claims for extra work:, contract defaults, together <br />with explanations of same; <br />2. Identification and description of any projects withi.n the previous five years <br />that the Prepares was deteimined by a pubiic entity not to be a responsible <br />construction manager, the reasons given by the public entity, together with an <br />explanation thereof; <br />3. An adequate demonstration of financial responsibi:lzty, which may include in <br />the Service Director's discretion, a certified financial statement prepared by a <br />certified public accountant, to assure that the Proposer possesses adequa.te <br />resources and availability of credit and the means a.nd ability to procure <br />insurance and bonds required for the project; <br />4. Disclosure of any suspension or revocations of an,y professional license of any <br />director, officer, owner, or mana.gerial employee af the proposed, to the extent <br />that any work to be performed is within the field af such licenses profession; <br />5. Disclosure of any and all OSHA violations within the previous three years, as <br />well as all notiees of OSHA citations filed against the proposed in.the same <br />three yeaz period, together with a description and explanation of remediation <br />or other steps taken regarding such viviations and notices of violation; <br />6. Disclosure of any and all violations within the previous $ve years pertaining <br />to unlawful intimidation or discrimination against any employee by reason of <br />race, creed, color, disability, sex, or na.tional origin and/or viola.tions of any <br />employee's civil labor rights or equa.t employment opportunities; <br />7.
