<br />ATTACHMENT 7.4 <br />NEW NORTH OLMSTED BRANCH LIBRARY <br />ADDITIONAL CRITERIA FOR DETERMINING THE <br />RESPONSIBILITY OF THE SURVEY'OR <br />In addition to the information requested in this Request for Proposal (RFP), each Surveyor shali <br />furnish the following information regarding the following items: <br />1. Documentation from previous projects regarding timeliness of performance, quality <br />of work, extension requests, fines and penalties imposed and payment thereof, liens <br />filed, history of claims for extra wark, contract defaults, together with explanations <br />of same; <br />2. Identification and description of any projects within the previous five years that <br />Surveyor was determined by a public entity not to be a responsible contractor, the <br />reasons given by the public entity, together with an. explanation thereof; <br />3. An adequate demonstration of financial responsibility, which may include in the <br />Service Director's discretion, a certified financial statement prepared by a certified <br />public accountant, to assure that the Surveyor possesses adequate resources and <br />avaiiability of credit and the means and abiIity tu procure insurance and bonds <br />required for the project; <br />4. Disclosure of any suspension or revocations of any professional license of any <br />director, officer, owner, or managerial employee of the Surveyor, to the extent that <br />any work to be performed is within the field of such licenses profession; <br />5. Disciosure of any and all O5HA violations within the previous three years, as well <br />as aIl notices of OSHA citations filed against the Surveyor in the same three year <br />period, together with a description and explanation of remediation or other steps <br />taken regarding such violations and notices of violation; <br />6. Disclosure of any and all violations within the previous five years pertaining to <br />unlawful intimidation or discriminatian against any employee by reason of race, <br />creed, color, disability, sex, or national origin and./or violations of any employee's <br />civil labor rights or equal employment opporiunities; - <br />Disclosure of any litigation (including copies of plead'zngs) in which the Surveyor <br />has been named as a defendant or third party defendant in an action involving a claim <br />for personal injury or wrongful death arising from performance of work related to <br />any project in which it has been engaged within th.e previous f ve years; <br />Disclosure of allegations of failure to pay wages, or unemployment insurance tax <br />delinquencies or unfair Iabor practices within the past five years; <br />Page 1 of 2
