APPENDlX E <br />New North Olmsted Branch Library <br />Additional Criteria for Determining the <br />Responsibility of the Proposer <br />ln addition to the priinary "r•esponsihrl.rfy yualificalrons" information requested in the <br />text of Part N. F of this Request for Proposal ("RFP), each Proposer shall furnish the <br />fallowing information regarding the following items. <br />Documentation from previous projects regarding trmeliness of performance, <br />quality of work, extension requests, fines and penalties imposed and payment <br />thereof, liens filed, history of claims for extra woric, contract defauits, together <br />with explanations of same; <br />2_ identification and description of any projects within the previous tive years <br />that the Prepares was determir?ed by a pubiic entity not to be a responsible <br />canstruction manager, the reasons given by the pubiic entity, tagether with an <br />explanation thereaf; <br />3. An adequate deriionatratian of financiai respansibitity, which may include in <br />the 5eivice Directar's discretion, a certified financial stateinent prepared by a <br />certified public accountant, to assure that the Proposer possesses adequate <br />resaurces and availability of credit and the means and ability ta procure <br />insurance and bonds required for the project; <br />4. Disclosure of any suspension or revocations of any professional license of any <br />director, officer, owner, or managerial einployee of`the proposed, to th.e extent <br />that aily wark to he perforn-ied is within the fie[d of'such licenses prafession; <br />5. Diselasure of any and all OSHA vioiations within 1:he prevrous three years, as <br />well as all notices of OSHA citations filed against rhe proposed in the same <br />three year period, tobether with a descriptian and explanation of remediation <br />or other steps taken regarding such violatians and riotices of violatzan; <br />6. Disclosure of any and aIi violations within the preNTious f ve years pertaining <br />to unlawful intimrdation or discrimination against any employee by reason of <br />race, creed, color, disabiiity, sex, or national origin and/or violations of an_y - <br />emplayee's civil iabor rights or equal emplayment opportunities; <br />7. Disctosure of any Iitigation (inciuding copres of pleadings) in which the <br />Proposer has been named as a defendant or third party defendant in an action <br />involving a claim for personal injury ar wrongfut death arising froi-n <br />performance of work related to any project in which it has been engaged <br />within the previaus five years; <br />Page 1 of 2
