Exhibit 1 Scope of Work The Independent Contractor shall conduct a safety belt education program to third grade students in performing services under this agreement. 2. The hzdependent Contractor will conduct a safety belt educational program for third grade students in any public, private, parochial, and in-home schools within its jurisdiction, using curriculum and informational materials provided by the ODPS. The program will use the formal classroom setting it1 school facilities during school hours. A classroom schedule will be forwarded to ODPS prior to any presentations to the schools. Upon completion of the classroom program, the Independent Contractor shall obtain from an authorized school official (classroom teacher or principal) the completed safety belt education evaluation form(s) for each classroom presentation. The completed evaluation forms must accompany invoices submitted to the ODPS. Under this agreement, the Independent Contractor will be reimbursed $4.00 for each third grader receiving instruction, not to exceed the amount identified on Page 2, Section 3. All instruction must be completed by May 1, 2009. 4. All new Independent Contractors participating in the Third Grade Safety Belt Program shal] provide ODPS with its formal written safety belt policy, which requires all employees to use safety belts while operating agency vehicles or personal vehicles when engaged in work activity for the contracting agency. The Independent Contractor will conduct a safety belt education program for its employees, to inform them of the benefits of safety belt usage. A statement shall be submitted to ODPS to verify that departmental employees have received safety belt education prior to forwarding the first invoice for payment. 5. The Independent Contractor may not accept any fees from any source other than ODPS for services conducted under this Agreement. The purchase of any type of equipment to implement this agreement is not authorized. For tne purpose of this project, the Independent Contractor shall be responsible to the ODPS/Ohio Traffic Safety Office (OTSO) Administratar or the assigned designee. The Independent Contractor acknowledges that all reports shall be submitted to the OTSO Administrator or the assigned designee and the directions for implementation of the defined activity shall be received from this authority. The ODPS will provide the cun•iculum materials, program evaluation forms, safety belt program literature and other educational materials to the Independent Contractor. ODPS will provide technical assistance to the Independent Contractor to implement this project. The ODPS shal] not reimburse the Independent Contractor for any travel expenses under this agreernent. For putposes of this agreement, the condition under Paragraph 2B of this agreement providing that Independent Contractor "shall furnish professiona] services performed according to applicable commercial standards" means the hidependent Contractor shall perform completely and accurately the work and activities described in this "Scope of Worlc", as well as meet the requirements under Paragraphs 1-4 above, di (rev. 9I16/2008) 7 of 8
