PROPOSAL DEADLINE <br />Three (3) bound copies of a proposal must be received by the City of North <br />Olmsted, 5200 Dover Center Road, North Olmsted, Ohio 44070, no later than <br />4;00 p.m., Monday, June 19, 2006. All proposals must be addressed to the <br />attention of Mr. Duane K. Limpert, Director of Public Service. A]1 proposals shall <br />be plainly marked on the outside "CITY ENGINEERING SERVICES <br />PROPOSAL." <br />V. PROFESSIONAL ENGINEERING SCOPE OF SERVICES <br />The successful Proposer will provide professional engineering scope of services as <br />outlined in APPENDIX B to this RFP. The services will be provided under the <br />Agreement between Owner and successful Proposer. <br />VI. CONTENTS OF PROPOSALS <br />Statements of qualifications must be included with the Proposal and should include <br />information regarding: <br />A. Firm's history; <br />B. Experience in providing professional engineering services for cities similar to <br />and larger than the City of North Olmsted; <br />C. Education and experience of staff who will be involved in the Project; <br />D. Firm's general liability insurance and financial responsibility; <br />E. List any pending or current legal proceedings such as mediation, arbitration <br />ar suits; <br />F. References; <br />G. Other services offered by the firm; and <br />H. Documentation indicating firm's compliance or non-compliance with each of <br />the standards (isted in Appendix B. <br />The Proposal shall include all information required to be submitted by the Request <br />for Proposal and any of its Appendices. The Proposer shall include a proposed <br />schedule of all services required as part of the professional engineering services. <br />Firms who wish to submit a proposal should follow the form of this Request for <br />Proposal. Please refer to Section VII - Subsection F for the specific requirements <br />for which the Proposer should use to document its qualifications since the specific <br />requirements will be used by the City of North Olmsted as the selection criteria in <br />evaluating the received Proposals. Please refer to Appendix B for the duties of the <br />successful Proposer. Please refer to Appendix C for the task and subtask <br />requirements for presenting the proposed professional services level of effort. <br />Please refer to Appendix D for the required format to be used by a Proposer in
