tours of duty or forty (40) hour Employee who is sick or disabled for five (5) consecutive <br />working days may be required to secure and submit a physician's release certifying that they are <br />fit to return to work. This release must be submitted to the Employee's Officer in Charge before <br />the Employee will be permitted to return to work. <br />15.8 Employees shall be permitted to contribute sick leave hours to a sick leave bank for use <br />by any employee who has exhausted all of his own sick leave under the following conditions: <br />A. The Sick Leave bank shall have a maximum balance of fifteen hundred (1500) <br />hours. <br />B. Employees that have suffered any illness or injury in the scope of their <br />employment and have exhausted the benefits of Article XVII or were not covered <br />by the provisions of Article XVII may draw from the sick leave bank after their <br />own sick leave bank has been exhausted. <br />C. Employees that have suffered any illness or injury to themselves or their <br />immediate family outside of the scope of their employment may draw from the <br />sick leave bank only after the employee has exhausted all of their own sick leave <br />bank, vacation and nolidays. <br />D. Any employee that contributes sick leave hours to the sick leave bank shall not <br />lose any sick leave bonus as granted by section.15.2. <br />E. No employee shall be germitted to contribute mare then one hundred (100) hours <br />duxing his terrn of employment. <br />F. Employees who have used hours from the sick leave bank shall be required to <br />repay the sick leave bank one (1) hour for every two (2) hours used. Employees <br />shall be required to repay a minimum of two (2) hours per pay period. Should an <br />emptoyee not be able to repay the sick leave bank prior to his separation from <br />service then t.he sick leave bank shall be credited from his separation check. Such <br />requirement of repayment nzay be waived by a majority vote of the Association's <br />Executive Board of Local 1267, the Chief and Assistant Chief. <br />15.9 The Ernployer may require any Employee requesting paid sick leave to furnish <br />substantiating evidence or a statement from his/her attending physician certifying that absence <br />from work was required due to one of the reasons set forth in 15.5. Such certification must be <br />presented whenever sick leave is requested for more than two (2) consecutive tours of duty for <br />Employees who work an average of 50.4 hours per week ancl for five (5) consecutive working <br />days for Employees on a forty (40) hour work week. <br />ARTICLE XVI <br />LEAVE OF ABSENCE <br />16.1 In addition to medical leaves to which they may be entitled to under the Family Medical <br />Leave Act ("FMLA"), employees may be entitled to take a personal leave of absence with the <br />18 <br />_--i
