reason. To be eligible for full payment, an employee shall be required to be certified for the <br />period between Decernber 15 and December 14, each year of this Agreement. <br />19.2 Employees shall be permitted to drop their paramedic certification after fifteen (15) years <br />of continuous service in order of seniority down. Employees that drop their Paramedic <br />certification must maintain their EMT certification. Employees shall be permitted to drop their <br />paramedic certification providing the total number of remaining paramedics does not fall below <br />thirty-three (33). Shauld manning increase, the total minimum number of paramedics required <br />would increase by one (1) for every three (3) additional hires. <br />ARTICLE XX HEALTH BENEFITS <br />20.1 Effective January 1, 2008, the Employer sha11 provide either individual or family medical <br />insurance coverage at the employee's option for each full-time employee pursuant to Appendix <br />A. The insurance coverage in effect in 2006 shall continue through 2007. <br />20.2 Effective January l, 2008 and through the duration of this agreement, employee(s) <br />participating within either an individual or family plan shall cantribute an amount equal to ten <br />(10%) percent of the plan premium or expected costs of such medical and vision plan and the <br />Employer shall contribute ninety (90%) percent of such costs. The employee contribution shall <br />be withheld via payroll deduction not later than the first pay period each month. <br />20.3 The City will offer a PPO plan that provides for annual deductibles, co-pays and co- <br />insurance as set forth in the attached Plan design for 2008, providing said plan contains <br />deductibles, co-pays, co-insurance benefits that are no less than those contained in Appendix A. <br />20.4 Employees shall participate in the AFSCME Benefit Plan at the present level of benefits. <br />1. Vision - Option Plan #1 (vision plan which was in effect August 1, 1994) <br />Vision - Option Plan #2 (vision plan sunilar to that offered through Union Eye <br />Care) <br />20.5 Expected cost of premium is defined as the cost of all medical, hospital, prescription drug <br />and related fees including, but not limited to, administration fees. <br />20.6 The Employer reserves the right to establish plans, enrollment periods and regulations. <br />20.7 All changes affecting the plan, providers' enrollment periods, and regulations shall be <br />considered and subject to recommendation by the healthcare committee in accordance with <br />Sectian 20.10. <br />20.8 All employees shall receive twenty-five thousand ($25,000.00) dollars in group term life <br />insurance, paid for by the Employer. <br />20.9 The City shall cause to be implemented the current I-3:ealth Caxe Plan as a qualified Plan <br />under Section 125 of the Internal Revenue Code. <br />22
