16.4 Written grievances must be filed on the form provided by the F.O.P. and shall <br />, <br />contain, but not be limited to, the following information. <br />1. Signature of the employee <br />16.5 The following steps shall be followed in the processing of a grievance. <br />Step 1. Within five (5) working days of the incident or knowledge of the incident (but in <br />no case later than thirty (30) calendaz days from the actual facts which give rise to the grievance, <br />the aggrieved employee shall submit his written grievance to the Shift Lieutenant, or his <br />designee, who shall indicate the date and time of receipt of the grievance, and affix his signature <br />to the grievance form. That Shift Lieutenant shall respond in writing to the grievant within f ve <br />(5) working days of receipt of the grievance. <br />Step 2. A grievance unresolved at Step 1 may be submitted by the grievant to the Chief of <br />Police, or his designee, within five (5) working da.ys from receipt of the Step 1 answer. It shall be <br />the responsibility of the Chief of Police, or his designee, to investigate the matter, hold such <br />hearings as necessary, and to provide a written response to the grievant within the (10) working <br />days of receipt of the grievance. The grievant may, at his option, be representeti by a <br />representative of the F.O.P. at any hearing or hearings heid at this level. <br />22 <br />... < . , ... , .
