_ t.. <br />SENIOR COMMUNITY SERVICE EMPLOYMENT PROGRAM <br />HOST AGENCY COMMLJNITY SERVICE & TRAI1vING AGREEMENT <br />November <br />This agreement is made and entered into this 2 7 th day of 2000 betw.een Senior Workers' Action Program, hereinafter <br />referred to as the SPONSOR AGENCY and City of North Olmsted and the North Olmsted City Schools <br />hereinafter referred to as the HOST AGENCY. <br />The HOST AGENCY is a: [?] governmental unit <br />OR an: [] IItS (501)(c)(3) tax exempt ageucy. <br />CHECK ONE of the above [], and PLEASE ATTACH a copy of your 501(c)(3) exemption letter if you are a NEW HOST AGENCY. <br />1. The PARTIES agree: <br />a) The period of agreement shall be from Julv 1. 2000 through June 30. 2001 . <br />2. The HOST AGENCY agrees: <br />a) To provide adequate supervision to enable the Trainee to perform as a productive and effective worker. <br />b) To provide orientation, instruction, and trainiag concerning the HOST AGENCY, its activities, the Trainee's day to day <br />responsibilities, and in prepazation of unsubsidized placement. <br />c) To send a Supervisor or representative to a group meeting of Host Agency Supervisors. <br />d) To assure that each Trainee is performing the duties outlined in tfie current Community Service Training Assignment <br />Description, and to notify the Program Representative to upgrade the Training Assignment before responsibilities change. <br />e) To provide safe and sanitary worldng conditions and any necessary liability coverage. <br />fj To ensure that the Trainee's activities will: <br />(1) Not result in the partial or totai displacement of currently employed workers, <br />(2) Not impair existing contracts or result in the substitution of Federal funds for other funds in connection with work <br />that would otherwise be done, <br />(3) Not be used as a replacement for a position from which a person has been laid off--including Maintenance of <br />Effort, <br />(4) One and only ONE TITI.E V NATIONAL CONTRACTOR will serve this agency. <br />g) Not to discriminate on the basis of race, color, religion, sex, national origin, handicap, age, political affiliation or opinion, <br />or ancestry. <br />h) To provide the SPONSOR AGENCY with time and attendance records that have been reviewed for accuracy & signed <br />by a documented Supcrvisor, as designated by the SPONSOR AGENCY, activity reports, and evaluations (when <br />requested), and accurate in-kind contribution records as required by the SPONSOR AGENCY. <br />i) To assure that each Trainee works ONLY the total number of hours authorized by the SPONSOR AGENCY. Volunteer <br />or overtime hours aze not permitted, unless, the HOST AGENCY agrees to pay the Trainee for the overtime hours. <br />Banking hours and Comp-time are prohibited. <br />j) To make no significant changes in a Trainee's work schedule, hours, training assignment duties, Supervisor, place of work, <br />or status within the HOST AGENCY without NOTIFICATION/APPROVAL of the SPONSOR AGENCY. <br />
