_?. <br />CITY OF NORTH OLMSTED <br />RESOLUTION NO. 2003 -147 <br />BY: Mayor Musial <br />A RESOLUTION AUTHORIZING THE MAYOR TO ENTER <br />INTO A CONTRACT WITH THE CUYAHOGA COUNTY <br />GENERAL HEALTH DISTRICT FOR THE YEAR 2004 <br />WHEREAS, the City of North Olmsted, under O.R.C. §3709.08, is a member of the <br />General Health District of Cuyahoga County; and <br />WHEREAS, the City of North Olmsted must enter into a contract for the year 2004 for <br />health services to be performed by the Cuyahoga County Health District; <br />NOW THEREFORE, BE IT RESOLYED BY THE COUNCIL OF THE CITY OF <br />NORTH OLMSTED, CUYAHOGA COUNTY, AND STATE OF OHIO: <br />5ECTION 1: That the Mayor be and he is hereby authorized to enter into a contract, a <br />copy of which is attached hereto and ma.rked Exhibit "A," with the Cuyahoga County General <br />Health District to provide health services to the City of North Olmsted from January 1, 2004 to <br />December 31, 2004. <br />SECTION 2: That a copy of this Resolution shall be forwarded to the Director of <br />Finance for authority to pay semi-annual payments under the provisions of this Resolution and <br />said contract. <br />SECTION 3: That this Resolution shall take effect and be in force from and after the <br />earliest period allowed by law. <br />PASSED: ? &tL) aa 3 <br />First Reading: _ <br />Second Rearding.• <br />Third Reading: <br />ir 3- o <br />I ? ??? a 3 <br />Committee: ,- 4- r k,_ <br />ATTEST.• <br />? , ''?? e.?,•-cc?-,?--- ??.i <br />44RBARA L.
